How do preclinical labs reconstruct serial whole-slide imaging into 3D volumes?

Serial sectioning and whole-slide imaging produce a stack of two-dimensional pathology slides. Three-dimensional reconstruction reveals structure that slide-by-slide review cannot: continuous vessel courses, immune microenvironments, and patterns of tumor infiltration across the specimen. You need a registered 3D volume to quantify those structures and to correlate histology with micro-CT or light-sheet from the same specimen.

What researchers typically do

Most digital pathology workflows stop at 2D: pathologists and researchers review individual whole-slide images in QuPath, Indica Labs HALO, Leica Aperio, or open-source viewers. That works for focal lesion review and stain scoring on a single plane. Reconstructing serial sections into 3D is often a separate custom pipeline: manual alignment of adjacent sections, interpolation between sparse levels, or research-only tools that do not connect to anatomic imaging from the same study. Multi-stain registration (H&E followed by IHC, for example) adds another alignment step per specimen. Labs pursue 3D reconstruction despite that friction because two-dimensional review misses continuous structure: how vessels course through tissue, how immune cells organize in three dimensions, and how tumors infiltrate across serial levels. Those questions require a registered volume, not isolated slides.
